I too received a plain white sleeve. I spoke with Tracy Petitt at Rizzoli who was involved with this book. She told me that in their description when they speak of the "record sleeve" they are talking about the cardboard insert in the box that holds the 45 rpm in the white sleeve. She said she is unaware of the picture sleeve and has never seen it. I sent her a link and the scan. She said that Johan was heavily involved in design of deluxe box along with Lou Reed. She said that he removed a small amount of singles from a few sets to take along to the UK for a signing. She "assumes" that he must have created the picture sleeve as some type of bonus to go with those few singles. She also is unaware of any way to get one of those sleeves. Looks like another "super-rarity" has been created. I had really hoped to try to get one thru Rizzoli but now that I understand the description on their site, I'm out of luck. Ps, I'm still pissed

The Steve Nelson cover art isn't pictured on either the record sleeve or cardboard insert, so I still don't understand the description on their site. The UK picture sleeve has a totally different image, so AFAIK it was never promised to be included in the deluxe edition.
