Thanks. I'm surprised the number is that low. They may have increased it to meet demand, hence the unsigned copies.

I too received a plain white sleeve. The artwork is in the book itself, but did any copies actually include it on the record sleeve?

Hang on a sec ... 'second album', or 'The Velvet Underground'?

More info on this sleeve for the 45 rpm.
http://www.factmagazine.co.uk/index.php ... &Itemid=26
above link has details of the book signing where this was available. Rizzoli has the following info on their site:
Deluxe Edition Special Features:
--Limited to 400 copies
--Book is encased in a windowed clamshell box featuring type design from a rare Velvet Underground concert poster
--Includes 7" 45rpm record with extremely rare versions of two Velvet Underground tracks. The tracks--"Real Good Time Together" and "If You Close the Door"--are taken from the personal collection of Mo Tucker and are produced by Universal Records. The record sleeve features an early version of the cover art for the band's second album "The Velvet Underground" drawn by friend of the band Steve Nelson as well as the track listing for the record.
--CDR with the same two rare tracks packaged inside the book allows for digital downloads of these collector's cuts.
I spoke with Tracy Petitt at Rizzoli who was involved with this book. She told me that in their description when they speak of the "record sleeve" they are talking about the cardboard insert in the box that holds the 45 rpm in the white sleeve. She said she is unaware of the picture sleeve and has never seen it. I sent her a link and the scan. She said that Johan was heavily involved in design of deluxe box along with Lou Reed. She said that he removed a small amount of singles from a few sets to take along to the UK for a signing. She "assumes" that he must have created the picture sleeve as some type of bonus to go with those few singles. She also is unaware of any way to get one of those sleeves. Looks like another "super-rarity" has been created. I had really hoped to try to get one thru Rizzoli but now that I understand the description on their site, I'm out of luck. Ps, I'm still pissed

The Steve Nelson cover art isn't pictured on either the record sleeve or cardboard insert, so I still don't understand the description on their site. The UK picture sleeve has a totally different image, so AFAIK it was never promised to be included in the deluxe edition.
