I suspect Paul Nelson posted on this very forum a few times under the name Hobokenlad. If not him, then it's obviously someone who was closely involved on the 1969 live album.
As for the 'Loaded' reference on the sleeve, remember that in '74 it was the Velvets' most recent album (other than Squeeze, of course!) and probably the most widely known.
